M-F 8-5

Must Haves:

Doctor of Pharmacy (PharmD) degree Active Pharmacist license in South Carolina

2+ years of clinical pharmacy experience (inpatient, ambulatory, or TOC preferred)

Experience with medication reconciliation and direct patient care

Strong communication and collaboration skills across multidisciplinary teams

Day-to-Day:

The Transitions of Care (TOC) Pharmacist plays a critical role in ensuring safe, effective, and efficient medication management as patients move between care settings. This position focuses on reducing medication errors, improving patient outcomes, and preventing readmissions through medication reconciliation, patient education, and interdisciplinary collaboration.

This role partners closely with inpatient pharmacists, providers, nursing, and case management teams to support seamless transitions from admission through discharge and post-discharge follow-up.

Key Responsibilities

Transitions of Care & Medication Management

• Perform comprehensive medication reconciliation at admission, transfer, and discharge
• Identify and resolve medication discrepancies, interactions, and therapy gaps
• Optimize medication regimens based on patient-specific factors and clinical guidelines
• Support “meds-to-beds” and discharge medication programs

Discharge & Post-Discharge Support

• Educate patients and caregivers on medication use, adherence, and side effects
• Coordinate timely discharge prescriptions and ensure medication access prior to discharge
• Conduct post-discharge follow-up (calls/clinic support) to address medication-related issues
• Identify barriers to adherence (cost, access, literacy, etc.) and implement solutions

Interdisciplinary Collaboration

• Work closely with physicians, nurses, and case management to support discharge planning and continuity of care
• Serve as a medication expert during rounds and care coordination discussions
• Partner with ambulatory and specialty pharmacy teams to ensure continuity across care settings

Quality & Outcomes Improvement

• Track and report TOC metrics such as readmissions, medication discrepancies, and patient adherence
• Participate in process improvement initiatives to enhance care transitions and reduce errors
• Ensure compliance with regulatory and accreditation standards

Clinical Support & Education

• Provide drug information and clinical recommendations to providers and care teams
• Educate staff on best practices related to medication reconciliation and transitions of care
• Contribute to development of protocols and workflows to improve TOC services

Required Qualifications
