Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A full-time Medication Therapy Management (MTM) pharmacist opportunity is available for a 13-week contract with a chance for direct hire. This role supports specialty MTM services in a dynamic environment, providing essential pharmaceutical care either onsite or remotely. Candidates must hold a current Louisiana pharmacist license and maintain licensure in at least five additional states from a specified list, ensuring compliance with multi-state practice requirements.

Work Schedule: Full-time hours

Key Responsibilities:

  • Prepare and dispense medications following physician orders and institutional protocols.
  • Accurately interpret verbal and written prescriptions and enter data into computerized medication profiling systems.
  • Maintain precise patient medication profiles and ensure accurate pharmaceutical dispensing.
  • Manage issuance and documentation of controlled substances according to legal requirements.
  • Monitor medication therapy for contraindications, interactions, allergies, and appropriate dosing.
  • Identify and report any suspected adverse drug reactions promptly.
  • Provide clinical consultation to healthcare practitioners, recommending cost-effective therapeutic alternatives as needed.
  • Document clinical activities and interventions meticulously.
  • Participate in quality improvement initiatives and medication use reviews.
  • Supervise pharmacy support personnel and verify their daily activities.
  • Organize work priorities to deliver timely pharmacy services with minimal supervision.
  • Conduct medication storage inspections and replace outdated or unusable drugs.
  • Respond professionally to telephone and direct inquiries.

Qualifications & Desired Experience:

  • Current Louisiana pharmacist license required.
  • Licenses in at least 5 states among Alaska, Arkansas, California, DC, Delaware, Hawaii, Indiana, Maine, Maryland, Massachusetts, Montana, New Hampshire, North Dakota, Oklahoma, South Dakota, Utah, Vermont, Virginia, West Virginia, Wyoming.
  • Prior retail or hospital pharmacy experience.
  • Ability to obtain licenses in 10 additional states within 18 months of hire.
  • Strong knowledge of medication therapy management and pharmacy operations.
