Job Description
We are seeking a Case Manager to support individuals in achieving housing stability and overall well-being. This role requires strong organizational skills, comfort managing a caseload of 20+ clients, and the ability to work effectively within harm reduction and trauma-informed care models.

Key Responsibilities
  • Interact with clients regularly and audit case files and paperwork to ensure accuracy and compliance
  • Meet with clients on a monthly basis to ensure connection to essential resources, including: Mental health services, Employment and job?sourcing support, Daily hygiene and basic needs resources
  • Assist clients with obtaining and replacing benefits and vital documents, including: State ID, Birth Certificate, Social Security Card
  • Provide employment assistance and facilitate housing referrals
  • Ensure all clients receive appropriate services and ongoing support to achieve and maintain physical health, mental health, and housing stability
  • Utilize harm reduction principles when supporting clients with complex and high-acuity needs
  • Demonstrate strong de-escalation and crisis intervention skills in challenging situations
  • Develop and maintain strong community resource relationships to support client needs
Required Qualifications
  • Minimum 1 year of experience as a Case Manager. Experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders
  • At least 1 year of direct HMIS documentation experience
  • Strong understanding of harm reduction, Housing First, and client-centered service delivery
  • Demonstrated experience with de-escalation, conflict resolution, and crisis response
  • Excellent organizational, time-management, and documentation skills
  • Ability to work effectively with vulnerable and diverse populations
  • Familiarity with local community-based resources and social service systems
  • Bilingual skills a plus (not required)
Shifts: Monday to Friday 8am-430am or Swing Tuesday- Saturday 3pm- 11:30pm

Pay Details: $23.00 to $26.00 per hour
