Hospice Licensed Practical Nurse (LPN)

We have recently engaged with a very progressive, well run and financially stable Hospice organization in the Atlanta area of Georgia. They are a family-owned organization that is committed to patient satisfaction, clinical excellence, and quality care for each member of the family.

Our client is offering the following benefits:

• Annual Salary: $65,000-$70,000
\n

• Mileage Reimbursement-$0.50 Per Mile

Responsibilities:

\n
• The LPN Will Maintain a Case Load of About 18 to 20 Patients Per Week
\n
• Provides Direct Patient Care and Assessments Based on a Plan of Care From the RN
\n
• Assess and Provide Patient/Family/ Caregiver Education and Information Pertinent to Diagnosis of Plan of Care
\n
• Provides Appropriate Pain/ Symptom Management
\n
• Performs Other Duties as Assigned by RN
\n
• Monday- Friday 8am to 5pm
\n
• Patients Per Day-5 on Average
\n
• Coverage Area- Gwinnett, Stone Mountain, Clarkston, Tucker, Chamblee and Lilburn
\n

Requirements:

\n
• LPN in the State of Georgia
\n
• 1 year of Hospice or Palliative Care Experience-Preferred
\n
• 2 years of in-Home Patient Care Experience-Required
\n
• Familiarity with EMR System KanTime-Preferred
