Registered Nurse (Home Health)
Job Description
Provide skilled home health nursing care in accordance with the physician's orders and the patient's plan of care. Responsible for patient assessment, care coordination, education, documentation, and ensuring compliance with Medicare, Medicaid, and agency standards.
Key ResponsibilitiesPerform comprehensive patient assessments, including OASIS admissions, recertifications, resumptions of care, transfers, and discharges.
Develop, implement, and revise individualized care plans.
Administer medications and treatments as prescribed.
Monitor patient condition and communicate changes to the physician.
Coordinate patient care with interdisciplinary team members.
Educate patients and caregivers on disease management, medications, and home care.
Supervise Home Health Aides and Licensed Practical Nurses (LPNs), including assigning duties and completing required supervisory visits.
Complete accurate clinical documentation and submit notes within required timeframes.
Participate in case conferences, discharge planning, quality improvement, and staff development activities.
Ensure compliance with agency policies, Medicare/Medicaid regulations, and accreditation standards.
Graduate of an accredited School of Nursing.
Current Registered Nurse (RN) license in the state of practice.
Home health or hospital nursing experience preferred.
Knowledge of Medicare/Medicaid regulations and OASIS documentation.
Current CPR certification (in-person certification required).
Valid driver's license, reliable transportation, and ability to travel to patient homes.
Strong clinical, communication, organization, and time management skills.
Successful criminal background check required.
Ability to travel to patient homes.
Ability to stand, walk, bend, lift, and assist patients as needed.
Sufficient vision, hearing, and manual dexterity to perform patient assessments and nursing procedures.
