- Contract Duration: 3 months
- Shift / Schedule: Day (5x8 Hours) 8:00 16:30

This is a travel nursing position in the Case Management department at an Acute Care Facility located in Apple Valley, CA. The role requires a seasoned traveler with demonstrated hospital-based acute care experience.
Key Responsibilities- Provide acute care case management and discharge planning services
- Manage patient caseloads at a ratio of 1:25 on weekdays and 1:45 on weekends
- Meet department needs including every other weekend coverage
- Minimum 2 years of Acute Care Case Management/Discharge Planning experience required
- Must have hospital experience; behavioral health experience does not apply toward this requirement
- Must be a seasoned traveler first-time travelers are not accepted (hard stop)
- Acute care case management and discharge planning skills
- Ability to manage high patient caseloads and meet department scheduling needs
- CA State RN License required (pending license accepted, but must be fully licensed by start date)

- Weekend requirement: every other weekend to meet department needs
- Floating Expectation: None
- RTO Restrictions: No more than 7 days
- COVID-19 Vaccination: Not required

This client is a healthcare and medical services organization operating an acute care facility in the Inland Empire region of Southern California.
