Position Overview
The Care Manager Assistant - LPN plays a vital role in facilitating the coordination of patient care and services. This position emphasizes high-quality care through a collaborative approach that involves coordinating, monitoring, and assessing services based on patient needs and interactions, all while adhering to relevant laws, regulations, and organizational standards.
Educational Background, Experience, and Licensing Criteria- Completion of a nursing diploma (either an associate's or bachelor's degree in nursing)
- Active and valid nursing license (LPN) in the relevant state, which must remain in good standing throughout employment
- Preferred: At least one (1) year of clinical experience in a post-acute care environment
- Preferred: Previous experience in case management, utilization review, and discharge planning
- Ability to produce written communication that is clear, concise, and well-structured
- Strong organizational skills with the ability to prioritize tasks and manage time effectively
- Contribute positively to the organization's goals and support its strategic initiatives
- Possess expert knowledge in the relevant field of practice
- Ability to make independent decisions when necessary
- Familiarity with the practices, procedures, and regulations governing operations in a post-acute care facility
- Implement and interpret departmental programs, goals, policies, and procedures
- Demonstrate proficiency in all competency areas, including general management duties, patient rights, and safety protocols
- Maintain professional relationships with colleagues, vendors, and other stakeholders
- Ensure confidentiality of all proprietary and sensitive information
- Adhere to organizational policies, including those related to harassment and compliance
- Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training
- Utilize relevant computer systems effectively, including data entry and retrieval
- Act as a liaison between external case managers, families, physicians, and community resources
- Provide training and education to patients, families, colleagues, and other care providers
- Implement standards of practice for care management, ethical performance, and care coordination
- Deliver exceptional customer service and maintain a positive demeanor towards patients
- Assist with the safe evacuation of patients when necessary
- Demonstrate reliable and consistent attendance
- Utilize reasoning skills and sound judgment in various situations
- Collaborate effectively within an interdisciplinary team
- Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving throughout the workday
- Read, write, speak, and comprehend the English language proficiently
