Day-to-Day:

The Ambulatory Registered Nurse (RN) provides high-quality, patient-centered care in a fast-paced internal medicine outpatient clinic. This role focuses on coordinating care for adult patients with acute and chronic conditions while partnering closely with providers and interdisciplinary teams to drive quality outcomes.

Key Responsibilities

Patient Care & Clinical Support

  • Conduct comprehensive patient assessments including history, vitals, and symptom evaluation
  • Triage patient concerns (in-person, phone, and electronic messages) and prioritize care needs
  • Administer medications, injections, and treatments per provider orders
  • Perform clinical procedures such as wound care, IV therapy, and specimen collection

Care Coordination

  • Collaborate with physicians, APPs, and care teams to develop and execute care plans
  • Coordinate referrals, diagnostic testing, and follow-up care
  • Support management of chronic conditions (e.g., diabetes, hypertension, COPD)

Patient Education & Engagement

  • Educate patients and families on diagnoses, medications, treatment plans, and preventive care
  • Reinforce care plans and support self-management strategies

Documentation & Compliance

  • Document all patient interactions, assessments, and interventions accurately in the EMR
  • Ensure compliance with clinic policies, regulatory requirements, and quality standards

Quality & Operations

  • Participate in quality improvement initiatives to enhance patient outcomes and clinic workflows
  • Support efficient patient flow and clinic operations
  • Maintain a safe, clean, and organized clinical environment

Qualifications

Required

  • Active Registered Nurse (RN) license (state or compact)
  • Current BLS certification
  • 1+ years of nursing experience (ambulatory or acute care)

Preferred

  • Prior experience in internal medicine, primary care, or outpatient setting
  • EMR experience (e.g., Epic)
  • Strong triage and care coordination skills
