**TITLE**
\n Registered Nurse – Home Health

**SCHEDULE**
\n Shift: 8 hours

**ASSIGNMENT DETAILS**
\n Contract Length: 20 weeks
\n Guaranteed Hours: 40

**REQUIREMENTS**
\n - Current state license as a Registered Nurse
\n - OASIS Experience
\n - Homecare Homebase Software Experience
\n - CHPN certification (Preferred)
\n - Proof of current CPR certification
\n - Valid driver's license
\n - Reliable transportation and insurance
\n - 1-2 years experience as a Registered Nurse in a clinical care setting
\n - 2+ years home health experience

**ROLES & RESPONSIBILITIES**
\n - Assess patients to identify their physical, psychosocial, and environmental needs.
\n - Develop and document the Plan of Care (POC) to ensure continuity of care.
\n - Educate patients and families regarding care, medications, and home safety.
\n - Supervise and provide clinical direction to support staff ensuring quality service delivery.
\n - Monitor patient pain levels and utilize appropriate pain management techniques.
\n - Maintain communication with interdisciplinary team members, patients, and families.
