RN Case Manager, (Local Contract to Hire) Mon - Fri, Days, Fort Worth, TX

This will be a temporary to hire role. This is an in person position.

Work onsite location: Fort Worth, TX 76107.

Work Schedule: Monday-Friday, 8am-5pm. Dress code: Scrubs.

• Required: experience in assessment skills, case management and care; EHR, technology proficiency -

typing skills, transportation, 2 years nursing experience

• Preferences: experience with IDD population, ICF or HSC group homes.

This is a Day Habilitation facility for adults with intellectual and developmental disabilities.
