Job Description
Job Title: Registered Nurse - Home Health
Location: Illinois
Salary: $80,000 - $85,000 annually
Responsibilities
Provide skilled nursing care during scheduled home visits.
Perform comprehensive patient assessments and ongoing evaluations.
Complete OASIS assessments and ensure timely, accurate clinical documentation.
Develop, implement, and update individualized plans of care.
Administer medications, treatments, wound care, and other skilled nursing services as ordered.
Monitor patient progress and communicate changes in condition to the physician and care team.
Educate patients and caregivers on medications, disease management, and home safety.
Coordinate care with physicians, therapists, and other healthcare professionals.
Ensure compliance with Medicare, CMS regulations, and agency policies.
Maintain accurate documentation using the agency's electronic medical record system.
Qualifications
Active Illinois Registered Nurse (RN) license.
Minimum of 3 years of home health nursing experience preferred.
Strong experience completing OASIS-E1 assessments.
Knowledge of Medicare home health regulations and documentation requirements.
Experience with Homecare Homebase (HCHB) is preferred.
Excellent clinical assessment, communication, and organizational skills.
Current BLS certification.
Valid driver's license, reliable transportation, and willingness to travel throughout the Chicagoland area.
