Job Summary
We are seeking an experienced RN Case Manager for a full-time direct hire opportunity serving Home Health and Hospice patients throughout the South Atlanta territory, including Stockbridge, Conyers, and surrounding areas. This field-based Registered Nurse Case Manager role is ideal for a compassionate RN who enjoys delivering meaningful, patient-centered care in the home setting while coordinating services that support comfort, quality outcomes, and continuity of care.

Key Responsibilities
- Conduct in-home patient visits, completing approximately 12 to 15 visits per week and 3 to 4 visits per day.
- Perform comprehensive nursing assessments and evaluate patient needs in the home health and hospice setting.
- Develop, implement, and update individualized plans of care based on patient condition, physician direction, and care goals.
- Coordinate care with physicians, interdisciplinary team members, patients, caregivers, and families.
- Maintain accurate, timely, and compliant clinical documentation using company-provided technology.
- Ensure care delivery aligns with applicable Home Health and Hospice regulations, standards, and best practices.
Compensation and Benefits
- Salary range: $88,000 to $98,000 per year.
- Full-time, direct hire opportunity.
- Flexible scheduling with availability required for 40 hours per week.
- Mileage reimbursement at $0.48 per mile.
- Laptop and company cell phone provided.

- Location: South Atlanta territory, including Conyers, GA to Stockbridge, GA and surrounding areas.
