Job
\n Summary

\n

Employee is responsible for the care if the residents in a
\n designated area of the facility. Employee communicates with off going and
\n arriving shifts of personnel to assure the continuum of quality of care. Makes
\n rounds, evaluating the physical, emotional and social needs of the residents
\n making sure these are being met through the staff assigned to work with each
\n group off residents. Assesses assigned staff’s ability to provide for each
\n residents needs and provides intervention as indicated. Supervises the care
\n provided by the Nursing Assistants assigned to the unit, providing intervention
\n and education as indicated. Make adjustments in assignments as indicated.
\n Prepares and administers medications and treatments according to procedures.
\n Consults with physicians, receives records and transcribes orders as required.
\n Makes rounds with physicians when necessary. Reviews resident’s care plans and
\n assures these plans are followed. Documents on records as per policy and
\n procedures of the facility. Participates in the evaluation of the employees
\n working under his/her supervision. Participates in staff meetings and
\n in-services as required.

\n

The Registered Nurse, Licensed Practical Nurse or Licensed
\n Vocational Nurse is responsible to the Director of Nursing and/or the Assistant
\n Director of Nursing.

\n

Education

\n

Employee must be a graduate of an accredited school of
\n nursing currently registered with the state agency for nursing licensure and
\n hold a valid nursing license in the state he/she is employed.

\n

Qualifications

\n

1.
Demonstrates leadership and managerial ability.

Has the ability to organize, plan and supervise
\n care and the Nursing Assistants assigned to work at a designated unit and
\n shift.

\n

\n

\n health, having sound judgement and high professional standards.

\n

Must be willing and capable of providing
\n emergency assistance to residents as indicated.

\n

Is courteous in working with residents, families
\n and employees.

\n

Maintains a net, well-groomed and professional
\n appearance at all times.

\n

8.
Has a desire to work with the geriatric
\n residents and their current and prospective needs.

\n

9.
Must be willing to be in the facility ready to
\n begin assigned duties on time.

\n

10. Must
\n be willing to be in the facility during weekdays as well as weekend hours.

\n

11. Must
\n be willing to work preferred shifts as scheduled and assist on other shifts as
\n needed to assure adequate staffing to meet the needs of each resident.

\n

12. Must
\n demonstrate ability to evaluate the geriatric resident’s needs and provide
\n intervention timely to assure each resident’s needs are met.

\n

13. Must
\n demonstrate ability to evaluate nursing staff needs and provide staff education
\n timely to provide quality care.

\n

14. Must
\n demonstrate ability to function as a cooperative team member with each member
\n of the Nursing Department and other departments.

\n

15. Must
\n demonstrate the ability to follow and supervise assigned staff to assure the
\n facilities policies and procedures are followed and upheld.

\n

16. Must
\n demonstrate the ability to manage and supervise the assigned unit in a
\n productive, timely manner.

\n

17. Must
\n have a pre-employment and an annual Tuberculosis screening examination.

\n

Responsibilities

\n

1.
Receives report from the off going shift.

\n

2.
Reviews communication systems to be
\n knowledgeable of any unusual occurrences during the previous shifts.

\n

3.
Assigns Nursing Assistants and other designated
\n personnel assuring each resident’s needs will be met during the assigned
\n shifts.

\n

4.
Gives report to Nursing Assistants.

\n

5.
Makes rounds of the assigned unit frequently
\n during the shift providing intervention as needed. During the entire shift,
\n will be attune to needs of unit while doing other nursing tasks.

\n

6.
Prepares and administers medications as ordered.
\n Reorders medications as needed in preparation for future shifts. Assesses and
\n documents medication issues as per policy.

\n

7.
Provides special procedures as ordered.

\n

8.
Completes the treatment as ordered. Assesses and
\n documents new and old areas of treatment describing in detail the results of
\n staff action and the of the treatment.
\n Prepares a weekly skin report for the Director of Nursing. Contacts the
\n physician for any areas not responding positively and timely.

\n

9.
Supervises the meal service for each resident on
\n the unit. Works with the staff to sure each resident is provided adequate nutrition
\n for each meal.

\n

10. Continually
\n assesses and reassesses residents for risk of complications as a result of
\n being in the facility. (i.e., skin breakdown, incontinence, contractures, reduced
\n mobility, reduced involvement in bathing, dressing, or eating, weight loss,
\n behaviors due to change in lifestyle, etc.)

\n

11. Continually
\n observes progress and quality of work of the Nursing Assistants to assure the
\n residents are turned, repositioned, toileted, walked, exercised, provided
\n foods, and fluids, well groomed, involved in their specific restorative
\n program, provides intervention as indicated, etc. Also observes the resident’s
\n environment to assure a clean and safe environment is provided for each
\n resident.

\n

12. Document
\n on nurse’s notes as per policy. Assures there is accurate and timely
\n documentation of any unusual observation, incident, or happening.

\n

13. Reviews
\n information, reassess residents, interviews staff, reviews documentation of
\n each shift and writes monthly nurse progress notes using the Interdisciplinary
\n Care Plan documenting progress or lack of progress toward the resident’s goals.
\n Writes suggestions for changes to be made to the plan of care on the “Working
\n Copy” of the Interdisciplinary Care Plan

\n

14. Admits
\n residents as required.

\n

15. Assists
\n in the ordering and reviewing of results of laboratory tests as indicated.
\n Notifies the physician of results.

\n

16. Notifies
\n the physician of unusual findings as indicated.

\n

17. Assists
\n with auditing charts.

\n

18. Assists
\n with answering and correcting consultant reports.

\n

19. Reviews,
\n signs and provides intervention as indicated for Nursing Assistants
\n documentation.

\n

20. Counts
\n required medications with oncoming and off going shifts.

\n

21. Makes
\n appropriate notes in Communication Books to assure adequate communication is
\n maintained for nursing.

\n

22. Assists
\n in keeping other departments informed of resident’s change of condition as
\n indicated.

\n

23. Assists
\n in providing in-services for assigned staff.

\n

24. Keeps
\n nurse’s station, medication room clean. Assigns Nursing Assistants to keep each
\n ancillary room clean and orderly. Assigns staff to keep equipment clean and
\n proper condition for use.

\n

25. Reviews
\n and updates Medication Administration Record, Treatment Administration Record,
\n and Nursing Care Flow Records for the upcoming month.

\n

26. Makes
\n rounds toward end of shift to assure area is in order for on coming shift.

\n

27. Gives
\n report to oncoming shift.

\n

28. Review
\n and assures there is adequate staffing for the oncoming shift. Replaces known
\n absences in a timely manner.

\n

29. Performs
\n any other tasks as assigned by the Director of Nursing or Assistant Director of
\n Nursing.

\n

Require full range of body motion including:

\n

1.
Standing/Walking: Continuously. Worker will be walking or standing almost
\n consistently during each tour of duty as they supervise, assist an access care
\n being provided.

\n

2.
Bending/Stooping: Continuously. Worker will be bending and stooping to
\n assist residents, co-workers, etc. Almost consistently during each tour of
\n duty.

\n

3.
Lift/Handling: Continuously. Worker will be assisting or totally lifting
\n residents as each resident needs, some residents needing to be totally lifted
\n or limbs may be lifted. Worker will be lifting various equipment for resident’s
\n needs, etc. Worker would be expected to lift, with assistance, up to 300 pounds
\n using equipment or other personnel to assist in the lifting.

\n

4.
Carrying: Continuously. Worker will be carrying equipment, etc. Range
\n of weight: 1-50 pounds.

\n

5.
Push/Pull: Frequently. Worker will be pushing or pulling medication
\n carts, equipment, etc. to provide care. Range of weight: 1-50 pounds.

\n

6.
Balancing: Frequently. Worker will be balancing when assisting
\n residents to transfer or transport, giving medication, doing treatment,
\n providing care, etc.

\n

7.
Twisting/Turning: Frequently. Worker will be twisting or turning when
\n providing medication and treatment systems, when assisting residents to
\n transfer or transport, assisting staff to provide for Activities or Daily
\n Living.

\n

8.
Crouching/Stooping: Frequently. Worker
\n will be crouching and stooping when providing medication and treatment systems,
\n when assisting staff to provide for A.D.L.s, gather equipment, provide care for
\n the residents, etc.

\n

9.
Kneeling: Occasionally. Worker will
\n be kneeling when providing medication and treatment systems, when assisting
\n residents to do A.D.L.s, etc.

\n

10. Reaching:
\n Worker will be reaching when providing
\n medication and treatment systems, when assisting residents to do A.D.L.s,
\n gathering equipment, providing care for the residents, etc.

\n

\n

11. Handling/Manual Continuously. Both fine and gross motor skills are needed
\n for all tasks throughout the working day.

\n

12. Speaking/Hearing: Continuously. Must communicate with residents, co-workers,
\n families, visitors and physicians continuously throughout tour of duty.

\n

JOB LOCATION
\n Must work throughout the nursing areas of the facility, May require
\n working irregular hours and may be exposed to communicable diseases and/or body
\n fluids, medical preparations, toxic substances and occasionally ionizing
\n radiation.
