- MD or DO degree.
- Board Eligible or Board Certified.
- Active and unrestricted medical license.

Experience and Certifications- Preferred: Over 5 years of experience in Family Practice.
- Preferred: Over 5 years of experience in Gynecology.
- Basic Life Support (BLS) certification is preferred.
- Required: Licensed Physician status.
- Required: Drug Enforcement Administration (DEA) License.
