Package Description: Care Co-ordinatorThe RoleGreat care isn't just about clinical treatment it's about making sure people receive the right support, at the right time, from the right services.As a Care Co-ordinator at IC24, you'll play a key role in helping patients navigate their healthcare journey, working closely with GPs, clinicians and wider community services to ensure care is joined up, personalised and centred around what matters most to each individual.We're looking for a compassionate, organised and proactive Care Co-ordinator to join our Primary Care team at Herstmonceux Integrative Health Centre. This is a rewarding opportunity to support patients with long-term conditions, complex needs and changing health circumstances, helping them access the services, information and support they need to live well and remain independent.At IC24, primary care is not delivered in isolation - it's connected.You'll work within an integrated healthcare system designed to improve access, reduce pressure on secondary care services and ensure patients receive the right care, in the right place, at the right time.What you'll focus on? Coordinating personalised patient careYou'll manage a caseload of patients, working closely with them, their families and carers to develop personalised care and support plans that reflect their individual needs, preferences and goals.? Supporting patients to navigate health and care servicesYou'll act as a central point of contact, helping patients access the most appropriate services, appointments and community support while ensuring they receive a seamless, joined-up experience across health and social care.? Empowering people to manage their own healthYou'll provide information, guidance and practical support that enables patients to better understand their conditions, access self-management programmes and make informed decisions about their care.? Working collaboratively across multidisciplinary teamsYou'll work alongside GPs, nurses, social prescribing link workers, health and wellbeing coaches and wider healthcare professionals to ensure patients receive coordinated, person-centred care.? Supporting multidisciplinary working and service deliveryYou'll help coordinate multidisciplinary team meetings, support palliative care and safeguarding processes, contribute to Quality and Outcomes Framework (QOF) activities and assist with practice initiatives that improve patient outcomes.? Building strong relationships with patients and carersYou'll provide compassionate, responsive support, ensuring patients and those who care for them feel informed, listened to and supported throughout their healthcare journey.? Contributing to continuous improvementYou'll support the ongoing development of primary care services by identifying opportunities to improve patient pathways, strengthen collaboration and enhance the overall patient experience.What matters in this roleWe're looking for someone who enjoys helping people and making a difference every day.You're likely someone who:Has experience working in health, social care or another patient-facing support roleDemonstrates excellent communication and interpersonal skillsIs highly organised and able to manage a varied workload while prioritising effectivelyHas experience working collaboratively within multidisciplinary teamsIs confident using Microsoft Office and learning other software.Can build positive relationships with patients, carers and healthcare professionalsIs compassionate, proactive and committed to delivering outstanding patient-centred careIs committed to continuous learning, teamwork and service improvementMost importantly, you're passionate about improving people's experiences of healthcare and helping patients access the support they need to live healthier, more independent lives.Who are we?We are Integrated Care 24 (IC24), the leading not for profit Social Enterprise providing innovative and patient focused primary care services.
