Job Overview
Under the guidance of the medical director, the Primary Care Physician (PCP) delivers essential medical services to participants of the PACE Organization (PO). This role involves assessing patient needs, collaborating with an interdisciplinary team to create care plans, providing direct medical care, and evaluating the effectiveness of these plans. The PCP also maintains a collegial relationship with other healthcare professionals, making independent decisions regarding medical requirements and health management, while performing necessary medical procedures.
Key Responsibilities- Engage as a member of the interdisciplinary team (IDT) during initial, semi-annual, unscheduled, and annual assessments; participate in daily morning updates and communicate any changes in participants' baseline conditions to relevant staff.
- Conduct initial and ongoing assessments of participants, develop care plans, and implement these plans. Ensure that the primary care treatment strategy aligns with the overall care plan established by the interdisciplinary team.
- Refer participants to medical specialists as needed and manage all aspects of care for participants residing in nursing homes. This includes regular visits to oversee their care in collaboration with the IDT and maintaining communication with nursing home staff for continuous monitoring.
- Admit nursing home participants to hospitals when necessary and oversee their use of medical specialists and inpatient services, ensuring effective communication and follow-up regarding the outcomes of specialist visits.
- Actively participate in interdisciplinary team meetings, sharing updates on participants, collaborating on care planning, and coordinating 24-hour care delivery.
- Review medical outcomes, statistical data, and utilization metrics under the direction of the Medical Director. Collaborate to identify issues and establish best practices to minimize unnecessary hospitalizations and enhance participant health.
- Complete necessary documentation and apply appropriate diagnostic coding.
- Advise the Medical Director on improving accountability of PO services to participants and referral sources, while keeping them informed about required resources as the program grows.
- Assist in developing policies, procedures, and standards of care; continuously monitor and evaluate patient care practices and service delivery; provide guidance and training to staff on medical and quality assurance matters.
- Doctorate Degree (MD or DO) is required, along with a current and active plenary physician license to practice in the state of PO.
- CDS Certification and DEA Registration are mandatory, as well as current BLS certification.
- Board Certification in Internal Medicine or Family Practice is required; Board Certification in Geriatrics is preferred.
- A minimum of one (1) year of experience working with frail or elderly populations is essential.
- A valid driver's license, proof of insurance, and reliable transportation are necessary.
- Must be proactive, possess business acumen, manage conflicts effectively, focus on customer service, demonstrate high-quality decision-making, and adapt to ongoing changes with organizational agility.
- Ability to work independently and exercise sound judgment is crucial.
- Strong interpersonal skills are needed to foster collaboration, commitment, and productivity within cross-functional teams, customers, and end users. Comfort in a virtual, collaborative leadership environment is essential.
- Exceptional written and verbal communication skills are required, with the ability to engage effectively with diverse audiences.
- Proficiency in the Microsoft Office Suite (Word, Excel, PowerPoint, etc.) is necessary, along with the ability to utilize other software as needed for job functions.
- Excellent organizational skills are required, with the ability to manage multiple tasks related to daily operations while maintaining high customer service standards. Must be able to handle frequent interruptions and adapt to changing priorities.
- Strong problem-solving abilities and critical thinking skills are essential, along with the capacity to prioritize workload effectively.
- This position requires occasional travel within the geographic region of the Centers.
