Position Summary

The Physical Therapist provides restorative and rehabilitative services as part of an interdisciplinary care team, helping elderly participants maintain independence, improve mobility, and enhance their quality of life. Working under the supervision of the Rehabilitation Supervisor, the Physical Therapist develops and implements individualized treatment plans in accordance with participants' overall plan of care.

Key Responsibilities

  • Collaborate with primary care providers and the interdisciplinary team to evaluate participants through muscle, nerve, joint, and functional assessments.

  • Develop and implement individualized physical therapy treatment plans to maximize safety, mobility, and independence.

  • Provide therapeutic interventions including therapeutic exercise, gait training, neuromuscular re-education, joint and soft tissue mobilization, functional training, and pain management modalities.

  • Conduct initial evaluations and periodic reassessments of participants.

  • Monitor participant progress, document outcomes, and communicate changes to the interdisciplinary team.

  • Educate participants, caregivers, and staff on therapy programs, mobility techniques, and the proper use of assistive devices such as wheelchairs, canes, braces, prosthetics, and orthotics.

  • Coordinate with vendors to obtain rehabilitation equipment and supplies.

  • Supervise and delegate appropriate patient care activities to Physical Therapy Assistants in accordance with California regulations.

  • Participate in interdisciplinary team meetings and contribute to individualized care planning.

  • Evaluate participants' home environments and recommend modifications or equipment to promote safety and independence.

  • Provide in-service education and training to healthcare team members.

  • Maintain accurate, timely clinical documentation in compliance with physical therapy practice standards.

  • Ensure therapy equipment is maintained in safe working condition and the department remains organized.

  • Follow infection control, safety, confidentiality, and professional practice standards.

  • Travel to multiple clinical sites and participant residences as needed.

  • Perform other duties as assigned.

Qualifications

  • Bachelor's degree or higher from a Physical Therapy program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE).

  • Current California Physical Therapist license in good standing.

  • Minimum one (1) year of experience working with the frail elderly population preferred.

  • Current CPR and First Aid certification.

  • Valid California driver's license, reliable transportation, and current vehicle insurance.

  • Ability to travel between multiple clinic locations and participant homes.

  • Ability to safely assist patients with transfers, mobility, and positioning.

  • Ability to lift, carry, push, or pull up to 25 pounds regularly and up to 50 pounds with assistance.

  • Strong communication, documentation, and interdisciplinary collaboration skills.

  • Experience in geriatric rehabilitation is highly preferred.
