Requirements:
  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree.
  • Successful completion of an accredited ACGME/AOA Internal Medicine residency and a Hematology Oncology fellowship.
  • Board Certification in Internal Medicine and Hematology Oncology, or the ability to obtain such certification within three years of employment.
  • Indiana medical licensure and privileges with our health system and affiliated hospitals as required.
  • Valid controlled substance registration with the Board of Pharmacy and DEA.
