Job Opportunity: Part-Time Primary Care Physician

Our organization is looking for a part-time physician specializing in Family Medicine, Internal Medicine, or Geriatric Medicine. This role involves providing care at Mercy LIFE Assisi House in Aston, PA, and Mercy Life Valley View in Elwyn, both situated in the greater Philadelphia area. The position requires a commitment of 20 hours per week.

Under the guidance of the medical director, the Primary Care Physician (PCP) will deliver essential medical services to participants of the PACE Organization (PO). Responsibilities include:

• Conducting assessments and collaborating with the interdisciplinary team to develop care plans.
• Providing direct medical care to participants and evaluating the effectiveness of care plans.
• Working collegially with other healthcare professionals and making independent medical decisions.
• Performing necessary medical procedures as required.

Position Overview

Mercy LIFE Assisi House serves as a retirement convent for the Sisters of St. Francis of Philadelphia, located near the congregation's motherhouse. This facility emphasizes a person-centered care approach, fostering a familial atmosphere within its 125-bed, multi-level care environment, which includes specialized units for Alzheimer’s and dementia care.

Mercy Life Valley View offers housing and the Mercy LIFE PACE program tailored for senior adults, specifically catering to a community of deaf seniors. Candidates should be open to learning American Sign Language.

The role entails working two 8-hour days at Assisi House and one 4-hour day at Valley View, with flexible scheduling during the week. Importantly, there is no requirement for night, weekend, on-call, or holiday coverage.

Key Responsibilities

• Engage as a member of the interdisciplinary team (IDT) for initial, semi-annual, unscheduled, and annual assessments, reporting any changes in participants' baseline status daily.
• Conduct initial and ongoing assessments of participants, develop and implement care plans, and integrate primary care treatment into the overall interdisciplinary care strategy.
• Refer participants to medical specialists as necessary and manage their care within the nursing home, ensuring regular visits and communication with nursing home staff for continuous monitoring.
• Oversee participants' interactions with medical specialists and inpatient care, following up on outcomes from specialist visits.
• Review medical outcomes to identify best practices and enhance participant health, ensuring thorough documentation and appropriate diagnostic coding.
• Advise the Medical Director on improving accountability of PO services and keep them informed about resource needs as the program grows.
• Assist in developing policies, procedures, and standards of care, while monitoring and evaluating patient care practices and service delivery.
• Provide guidance and training to staff on medical and quality assurance matters.
• Participate in the provider on-call rotation as required.

Minimum Qualifications

• Doctorate Degree (MD or DO) with a current and active plenary physician license to practice in the state.
• CDS Certification and DEA Registration are mandatory.
• Current BLS certification is required.
• Board Certification in Internal Medicine or Family Practice is essential; Board Certification in Geriatrics is preferred.
• Willingness to learn American Sign Language is necessary.
• A minimum of one year of experience working with frail or elderly populations is required.
• A valid driver’s license, proof of insurance, and access to a personal vehicle are essential.
• Strong communication, problem-solving, and organizational skills are crucial, along with the ability to effectively engage with diverse groups.
• Proficiency in Microsoft Office Suite is expected.
• Ability to adapt to changing priorities and demonstrate critical thinking skills while managing workload effectively.
• Periodic travel within the Center's geographic area may be necessary.
