Position Overview

Our Client is seeking an experienced, compassionate, and dedicated Physician Assistant (PA) or Family Nurse Practitioner (FNP) to join their growing Primary Care team serving Skilled Nursing Facilities in Jackson, CA, Marysville, CA, and Yuba City, CA.

This position offers both full-time (5 days/week) and part-time (3 days/week) opportunities for providers who are passionate about delivering exceptional care to residents in long-term care settings while working within a collaborative multidisciplinary healthcare team.

Key ResponsibilitiesPrimary Care & Patient Management
  • Provide comprehensive primary care services to Skilled Nursing Facility residents.

  • Perform patient evaluations, physical examinations, diagnoses, treatment planning, and follow-up care.

  • Develop individualized treatment plans based on patient needs and medical history.

  • Monitor patient progress and modify treatment plans when clinically appropriate.

  • Manage acute and chronic medical conditions while promoting preventive healthcare.

Patient Education
  • Educate patients and family members regarding diagnoses, medications, treatment plans, preventive care, and overall health maintenance.

  • Provide compassionate counseling to improve patient understanding and treatment compliance.

  • Encourage healthy lifestyle choices and disease prevention strategies.

Clinical Documentation
  • Maintain complete, accurate, and timely medical records using Electronic Health Record (EHR) systems.

  • Document patient encounters, assessments, treatment plans, medications, and follow-up recommendations.

  • Ensure documentation complies with state regulations and organizational standards.

Collaboration & Quality Improvement
  • Work collaboratively with physicians, nurses, therapists, administrators, and interdisciplinary healthcare teams.

  • Participate in quality assurance initiatives, peer review activities, and clinical performance improvement programs.

  • Support continuity of care through effective communication with facility staff and healthcare providers.

Regulatory Compliance
  • Deliver care in accordance with California Medical Board and Nursing Board regulations.

  • Follow all organizational policies, Skilled Nursing Facility protocols, and contractual requirements.

  • Maintain compliance with HIPAA, patient confidentiality standards, and all applicable healthcare regulations.

Required QualificationsEducation
  • Graduate of an accredited Physician Assistant (PA) or Family Nurse Practitioner (FNP) program.

Licensure & Certifications
  • Active California PA or NP License.

  • Board Certification.

  • Valid DEA Registration.

  • Current CPR/AED Certification.

Experience
  • Previous Primary Care experience preferred.

  • Experience working in Skilled Nursing Facilities, Long-Term Care, or Geriatric Medicine is highly preferred.
