Our organization is excited to announce an opportunity in the rapidly growing field of Sub-Acute and Long-Term Care, offering a remarkable work-life balance with no on-call duties or weekend shifts! We are looking for a full-time provider (40 hours per week) to join our Casa Real facility in Santa Fe, NM. This role is open to both nurse practitioners and physician assistants, and we encourage new graduates to apply.
The salary for this position ranges from USD $115,000.00 to USD $125,000.00 per year, along with an incentive compensation bonus program that has no cap.

Key Responsibilities- The Advanced Practice Provider will report directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager.
- Collaboration with the Center Leadership Team will be essential to meet center-specific goals and priorities.
- Work alongside the Medical Director and/or Attending Physician to deliver patient-specific medical care.
- Provide primary and urgent care services to residents in the nursing center, working in conjunction with other medical providers.
- Partner with Center Leadership to fulfill our mission of enhancing lives through high-quality healthcare and compassionate service.
- The Advanced Practice Provider must have graduated from an accredited nursing program and hold a license as a registered nurse. Certification as a Nurse Practitioner is required, with a Master's Degree in Nursing focusing on Older Adult and/or Family Nurse Practitioner (FNP) specialties. Other concentrations may be considered based on experience and organizational needs.
- For Physician Assistants, graduation from an ARC-PA accredited program is necessary, along with passing the PA National Certifying Exam (PANCE) to obtain the PAC designation. State licensure and NCCPA certification are also required.
- A minimum of two years of experience working with adult or geriatric populations is preferred.
- In states where permitted, APRNs must possess an unrestricted professional license and DEA registration, as must PAs.
