Job Title: Case Manager (Roxbury, MA)

Overview:

  • The Case Manager will recruit and engage HIV positive, hard-to-reach clients into support services. Support clients in building self-efficacy to manage their health by maintaining adherence to medical care and treatment, mental health services, and other HIV support services as needed.

Essential Functions:

  • Facilitating access to services for clients who are newly diagnosed, sporadically in care or lost to care, by conducting outreach within social networks, at internal HIV screening services and other clinics, testing sites, or through other mechanisms.
  • Provide emotional support in coping with HIV diagnosis and managing care.
  • Advocating for clients within the agency, meeting with external Case Managers and other relevant providers to share understanding of client's needs.
  • Supporting clients to remain in care by helping to navigate the medical and social service delivery system and assisting clients to build capacity to manage their care coordination.
  • Helping clients develop harm/risk reduction strategies for a variety of sexual and drug use behaviors and providing ongoing reinforcement of theses.
  • Strict adherence to MAC's protocols and all city, state and federal HIPAA law

Responsibilities:

  • Refer clients to in house services i.e. HEPC/STI/COVID testing and counseling
  • Conduct outreach to various venues to engage new clients
  • Enroll clients and update files as necessary
  • Implement individual and group Health Education/Risk Reduction interventions.
  • Participate in agency meetings, program planning, and strategic planning processes.
  • Ensure client confidentiality and maintain secure client files.
  • Participate in professional development, training, workshops and/or courses to increase individual capacity and industry awareness
  • Other duties as assigned

Minimum Education and Experience Requirements:

  • High School diploma

  • Must have 1-2 years of Case Management experience

  • Bilingual with Spanish

Schedule/ Shift:

Monday, Tuesday, Thursday and Friday 10am - 5pm

Wednesday 12pm - 7pm
