Job Description
Our organization is currently looking for a full-time Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to become a vital member of our clinic team in Jacksonville, FL.
As a Nurse Practitioner (NP) or Physician Assistant (PA) at our facility, you will collaborate with a diverse group of professionals, including physicians, pharmacists, care coaches, and behavioral health specialists, all dedicated to providing personalized, high-quality care.

You will report directly to the Associate Medical Director and work at our clinic located on Phillips Hwy, FL.

- Oversee referrals, coordinate with hospitals/SNF, and manage DME and home health services.
- Document patient care with assistance from quality-focused coders.
- Participate in on-call rotations and contribute to the growth initiatives at your center.
- Have experience in providing value-based care.
- Work on-site at the clinic.
Essential Qualifications:
- Master’s degree in nursing with board certification.
- Active and unrestricted NP or PA license in Florida.
- Experience in value-based care, primary care, or Medicare Advantage environments.
- Possession of an active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.
- Knowledge of HEDIS quality indicators and Medicare guidelines.

Compensation Range: The salary range provided below represents a good faith estimate of the starting base pay for full-time (40 hours per week) employment at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.
$123,800 - $170,300 per year
This position is eligible for a bonus incentive plan based on individual and/or company performance.
