Nurse Practitioner or Physician Assistant

Our organization is currently looking for a full-time Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to become a vital member of our clinic team in Jacksonville, FL.

As a Nurse Practitioner (NP) or Physician Assistant (PA) at our facility, you will collaborate with a diverse group of professionals, including physicians, pharmacists, care coaches, and behavioral health specialists, all dedicated to providing personalized, high-quality care.

You will report directly to the Associate Medical Director and work at our clinic located on Phillips Hwy, FL.

• Collaborative Care Model: Work alongside a dedicated team focused on supporting the physical, emotional, and social well-being of patients.
• Increased Patient Interaction: Manage a smaller patient load to allow for more meaningful interactions and care.
• Supportive Environment: We cultivate an inclusive culture that prioritizes teamwork and professional growth.

• Deliver comprehensive care to senior patients in accordance with clinical standards.
• Engage daily with your care team to ensure coordinated and high-quality service.
• Oversee referrals, coordinate with hospitals/SNF, and manage DME and home health services.
• Document patient care with assistance from quality-focused coders.
• Participate in on-call rotations and contribute to the growth initiatives at your center.
• Have experience in providing value-based care.
• Work on-site at the clinic.

Essential Qualifications:

• Master’s degree in nursing with board certification.
• Active and unrestricted NP or PA license in Florida.
• Experience in value-based care, primary care, or Medicare Advantage environments.
• Possession of an active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.
• Knowledge of HEDIS quality indicators and Medicare guidelines.

Scheduled Weekly Hours: 40

Compensation Range: The salary range provided below represents a good faith estimate of the starting base pay for full-time (40 hours per week) employment at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.

$123,800 - $170,300 per year

This position is eligible for a bonus incentive plan based on individual and/or company performance.
