Working under the guidance of the medical director, the Primary Care Physician (PCP) delivers essential medical services to participants of our organization. This role involves assessing patient needs, collaborating with an interdisciplinary team to create care plans, providing direct medical care, and evaluating the effectiveness of these plans. The PCP also collaborates with other healthcare professionals, making independent decisions regarding patient care and executing necessary medical procedures.

Key Responsibilities:

  • Engage as a member of the interdisciplinary team (IDT) by participating in initial, semi-annual, unscheduled, and annual assessments; attend daily morning updates and communicate any changes in participants' baseline conditions to relevant staff.
  • Conduct initial and ongoing assessments of participants, develop care plans, and implement these plans. Ensure that the primary care treatment plan aligns with the overall care strategy established by the interdisciplinary team.
  • Refer participants to medical specialists as necessary. Oversee the care of participants in nursing homes, conducting regular visits in collaboration with the interdisciplinary team, and maintain communication with nursing home staff for ongoing monitoring; admit nursing home participants to hospitals when required.
  • Manage and oversee participants' interactions with medical specialists and inpatient care, ensuring effective communication and follow-up regarding the outcomes of specialist visits.
  • Actively participate in interdisciplinary team meetings, maintaining regular attendance and contributing to discussions about participant changes, care planning, and coordination for continuous care delivery.
  • Under the direction of the Medical Director, review medical outcomes, statistical data, and utilization metrics, including the necessity of hospitalizations and specialist referrals. Collaborate with the Medical Director to identify issues and establish best practices, working with clinical staff and the interdisciplinary team to implement procedural or operational changes aimed at reducing unnecessary hospitalizations and enhancing participant health.
  • Ensure accurate documentation and appropriate diagnostic coding.
  • Advise the Medical Director on strategies to enhance accountability of services to participants and referral sources, keeping the Medical Director informed about necessary resources as the program grows.
  • Assist in developing policies, procedures, and standards of care; conduct ongoing monitoring and evaluation of patient care practices and service delivery; provide guidance and training to staff on medical and quality assurance matters.

Minimum Qualifications:

  • Doctorate Degree (MD or DO) is required, along with a current and active physician license to practice in the state. CDS Certification and DEA Registration are mandatory, as well as current BLS certification.

  • Board Certification in Internal Medicine or Family Practice is required; Board Certification in Geriatrics is preferred.

  • A minimum of one (1) year of experience working with elderly or frail populations is required.

  • A valid driver's license, proof of insurance, and reliable transportation are necessary.

  • The candidate must be proactive, possess business acumen, manage conflicts effectively, focus on customer service, demonstrate high-quality decision-making, adapt to change, and exhibit organizational agility. The ability to work independently and exercise sound judgment is essential.

  • Strong interpersonal skills are required to foster collaboration, commitment, and productivity when working with diverse teams, customers, and end-users. Comfort in a virtual, collaborative leadership environment is essential.

  • Exceptional written and verbal communication skills are necessary, with the ability to convey information effectively to various audiences.

  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, etc.) is required, along with the ability to utilize other software necessary for job functions.

  • Excellent organizational skills are essential, with the ability to manage multiple tasks related to daily operations while maintaining high levels of customer service. The candidate should be capable of handling frequent interruptions and adapting to changing priorities. Strong problem-solving abilities are also necessary.

  • Critical thinking skills and the ability to prioritize tasks effectively are required.

  • This position may require occasional travel within the geographic region of the organization.
