Position Summary and Requirements- Evaluate home care patients to identify their physical, psychosocial, and environmental needs, as documented through clinical records, case conferences, team reports, call logs, and on-site assessments.
- Complete OASIS assessments and visit documentation in accordance with agency policies, ensuring clinical notes reflect ongoing communication and coordination with physicians, interdisciplinary team members, and patients/families/caregivers.
- Report significant findings, issues, and changes to the Clinical Manager and physician, documenting all observations, communications, and relevant interventions.
- Oversee and provide clinical guidance to home health aides and LPNs/LVNs to maintain quality and continuity of care.
- Participate in on-call rotations and respond to emergency calls as per agency guidelines.
- Basic Cardiac Life Support certification must be obtained within 30 days of starting employment.
- Valid Driver's License.
- Registered Nurse (RN) license.
- Associate Degree in Nursing.
