Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

An opening position is available for an Infusion/Clinical Pharmacist in Memphis, TN with a direct hire opportunity. This role focuses on preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications within a home infusion setting. The pharmacist will serve as a critical member of a multidisciplinary team to ensure safe and effective treatments for patients.

Schedule and On-Call:

  • Monday through Friday, 8 a.m. to 5 p.m.
  • Participate in an on-call rotation, averaging fewer than 5 after-hours calls per week

Key Responsibilities:

  • Prepare sterile compounded pharmaceuticals and IV solutions in compliance with state and federal regulations
  • Assess lab values and calculate pharmacokinetic doses, including vancomycin and Total Parenteral Nutrition (TPN) adjustments
  • Provide detailed patient counseling on specialty medications and treatment plans
  • Educate patients and caregivers on infusion pump use, sterile techniques, and catheter maintenance at home
  • Coordinate care with prescribing physicians, home health nurses, and internal teams to optimize therapy management and prevent hospital readmissions
  • Maintain regulatory compliance with State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant standards
  • Manage pharmacy inventory and ensure safe, timely shipment of medications and supplies to patients

Qualifications and Desired Experience:

  • Minimum of 3 years’ experience in infusion specialty pharmacy
  • IVIg and hemophilia therapy experience are highly preferred
  • Solid knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations
  • Current, active Tennessee pharmacist license in good standing
  • Familiarity with CareTend pharmacy system is a plus
