Overview of the Role and QualificationsThis position involves delivering coordinated nursing care to patients across various age groups in their homes. You will be accountable for collaborating with an interdisciplinary team to set and achieve patient objectives while ensuring high standards of care. Your work will be guided by physician orders and overseen by the Clinical Manager.
Key Responsibilities:
- Evaluate home care patients to identify their physical, psychosocial, and environmental needs, as documented in clinical records, case conferences, team reports, call logs, and on-site assessments.
- Complete OASIS assessments and visit documentation in line with agency policies, ensuring clinical notes reflect ongoing communication and coordination with physicians, team members, and patients/families/caregivers.
- Report significant findings, issues, and changes to the Clinical Manager and physician, documenting all communications and interventions appropriately.
- Provide supervision and clinical guidance to home health aides and LPNs/LVNs to maintain quality and continuity of care.
- Participate in on-call rotations and respond to emergency calls as per agency guidelines.
- Basic Cardiac Life Support certification must be obtained within 30 days of starting employment.
- Registered Nurse (RN) license.
- Valid Driver's License (DL).
- Associate or Bachelor's Degree in Nursing.
Minimum of 1 year of relevant experience is required.
