Key Responsibilities

• Conduct assessments of residents' mobility and functional capabilities.
• Create personalized treatment plans based on individual needs.
• Provide evidence-based therapies to promote physical well-being.
• Work collaboratively with nursing and caregiving staff to ensure comprehensive care.
• Document all care activities in the electronic health record (EHR) system.
• Educate residents and their families on safety practices and home exercise routines.

Qualifications and Requirements

• Valid Physical Therapy license.
• Experience in geriatric care is preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively within a team-oriented environment.
