Key Responsibilities

• Assess residents' physical conditions and needs.

• Work collaboratively with nursing staff and care teams to ensure comprehensive resident care.
• Educate residents and their families on safety measures and fall prevention strategies.
• Maintain accurate documentation of care using electronic medical record (EMR) systems.

Qualifications and Requirements

• Valid Physical Therapy license.
• Experience in geriatric care is preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
