Description:

We are currently recruiting experienced Band 6 Community Psychiatric Nurses (CPNs) to join the Recovery & Wellbeing Team at the Taylor Centre in Southend. This is an excellent opportunity for skilled mental health professionals who are experienced in community mental health and care coordination.

Key Responsibilities:

\n
• Manage a caseload of up to 35 patients with severe and enduring mental health conditions
\n
• Act as Care Coordinator in accordance with the Care Programme Approach (CPA)
\n
• Complete comprehensive mental health assessments and risk assessments
\n
• Develop, implement, and review person-centred care plans
\n
• Work collaboratively with the multidisciplinary team and external agencies
\n
• Provide therapeutic interventions and ongoing support to service users
\n
• Maintain accurate clinical records and documentation in line with NHS policies
\n
• Promote recovery, independence, and positive mental health outcomes
\n

Requirements:

\n
• Valid NMC Registration (Registered Mental Health Nurse)
\n
• Previous Community Psychiatric Nursing (CPN) experience
\n
• Experience managing a community mental health caseload
\n
• Knowledge of CPA, risk assessment, and safeguarding procedures
\n
• Up-to-date mandatory training
\n
• Enhanced DBS
\n
• Available to start ASAP
\n
