\n\nDescription:
\nWe are currently recruiting experienced Band 6 Community Psychiatric Nurses (CPNs) to join the Recovery & Wellbeing Team at the Taylor Centre in Southend. This is an excellent opportunity for skilled mental health professionals who are experienced in community mental health and care coordination.
\n\nKey Responsibilities:
\n- \n
- Manage a caseload of up to 35 patients with severe and enduring mental health conditions \n
- Act as Care Coordinator in accordance with the Care Programme Approach (CPA) \n
- Complete comprehensive mental health assessments and risk assessments \n
- Develop, implement, and review person-centred care plans \n
- Work collaboratively with the multidisciplinary team and external agencies \n
- Provide therapeutic interventions and ongoing support to service users \n
- Maintain accurate clinical records and documentation in line with NHS policies \n
- Promote recovery, independence, and positive mental health outcomes \n
Requirements:
\n- \n
- Valid NMC Registration (Registered Mental Health Nurse) \n
- Previous Community Psychiatric Nursing (CPN) experience \n
- Experience managing a community mental health caseload \n
- Knowledge of CPA, risk assessment, and safeguarding procedures \n
- Up-to-date mandatory training \n
- Enhanced DBS \n
- Available to start ASAP \n
