In a retail or community pharmacy environment, the Pharmacist interprets and dispenses both prescription and over-the-counter medications to patients. They actively address prescription-related issues and offer advice to healthcare providers regarding medication selection, dosage, interactions, and potential side effects.

Additionally, the Pharmacist counsels patients on their prescriptions, covering aspects such as administration, possible side effects, expected outcomes, storage, and any inquiries from the patient. Monitoring patient health and progress in relation to drug therapy is also a key responsibility. The role requires adaptability to new technologies as the pharmacy profession evolves and includes providing oversight and clinical supervision for pharmacy staff.

In a medical center or institutional context, the Pharmacist interprets provider orders, offers drug information, assists with problem-solving, and engages in various aspects of pharmaceutical care. Responsibilities may include unit dose dispensing, IV admixtures, outpatient prescriptions, navigating electronic medical records, compounding, and utilizing automated dispensing systems. The Pharmacist may also serve as a shift supervisor under the guidance of department managers and must demonstrate effective written and verbal communication skills.

Pharmacy services may encompass areas such as acute care surgery, neurology, cardiology, pediatrics, oncology, infusion services, intensive care, general medicine, dialysis, transplant, antimicrobial stewardship, and medication reconciliation. Responsibilities may include decentralized clinical functions like direct patient interaction, interdisciplinary team rounding, chart reviews, and accurate medication list collection, as well as centralized functions such as medication dispensing and overseeing IV room operations.
