About the Role
The Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC) will provide clinical assessment, behavioral health planning, care coordination, and therapeutic support as part of a multidisciplinary care team. This role supports the development, implementation, monitoring, and coordination of behavior management plans consistent with patient-centered care goals.
Responsibilities
Provide direct clinical services and maintain therapeutic relationships with patients as needed.
Deliver behavioral health services to children in a school-based setting.
Coordinate care and ensure effective communication among clinical team members, patients, families, and referral partners.
Participate in quality improvement and performance improvement initiatives.
Assess psychosocial factors and provide relevant information to physicians and healthcare team members to support diagnosis and treatment planning.
Provide education and support to patients and families regarding psychiatric treatment, care plans, and ongoing services.
Connect patients and families with appropriate community resources.
Assist patients in understanding, accepting, and following medical recommendations.
Provide consultation, education, and training to staff regarding mental health and substance use concerns.
Maintain accurate documentation of progress, treatment plans, and service delivery in the electronic medical record (EMR).
Participate in clinical meetings and collaborate with interdisciplinary teams.
Prepare psychosocial assessments and reports as part of individualized treatment plans.
Provide developmentally appropriate care based on patient age, needs, and stage of development.
Maintain current clinical knowledge and professional practices.
Perform additional duties and responsibilities as assigned.
Qualifications
Education & Experience
Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC).
Minimum of three years of professional clinical experience preferred.
Current state license or eligibility for licensure.
Valid driver's license required.
Work Environment
This position works in a professional clinical environment and requires frequent interaction with patients, families, and healthcare professionals. The role involves managing multiple responsibilities while utilizing standard office technology and electronic documentation systems.
Physical Requirements
The employee must be able to perform essential job functions with or without reasonable accommodation. Duties may include sitting, standing, walking, speaking, hearing, keyboarding, and occasional lifting or movement of supplies or equipment.
