Employment Type: Part-Time (20-25 Hours/Week)

Location: Maricopa County, Greater Phoenix Metro Area

Position Summary

We are seeking a compassionate and dedicated Speech Language Pathologist (SLP) to provide high-quality speech and language therapy services in patients' homes. This part-time opportunity offers a flexible weekday schedule and the chance to work with a diverse patient population in their home environment.

Key Responsibilities

Evaluate, diagnose, and treat speech, language, communication, cognitive, voice, and swallowing disorders.

Conduct comprehensive assessments and develop individualized treatment plans based on each patient's needs, goals, and physician orders.

Provide skilled speech therapy services in patients' homes while adhering to evidence-based clinical practices.

Monitor patient progress, reassess functional outcomes, and modify treatment plans as needed.

Educate patients, families, and caregivers on therapeutic techniques, communication strategies, and home exercise programs.

Collaborate with physicians, nurses, physical therapists, occupational therapists, case managers, and other healthcare professionals to coordinate patient care.

Maintain accurate, timely, and compliant clinical documentation, including evaluations, progress notes, treatment records, and discharge summaries.

Ensure compliance with all applicable regulatory and organizational standards.

Participate in interdisciplinary care planning and contribute to positive patient outcomes.

Promote patient safety, independence, and quality of life throughout the course of treatment.

Qualifications

Master's degree in Speech-Language Pathology from an accredited program.

Active Speech Language Pathologist license, or eligibility for licensure in the state of practice.

Minimum of one (1) year of experience as a Speech Language Pathologist.

Additional state-required certifications or credentials as applicable (assistance may be provided to obtain required certifications).

Current CPR/BLS certification, if required.

Valid driver's license, reliable transportation, and automobile insurance for travel between patient homes.

Preferred Qualifications

Previous home health experience.

Experience working with adult and geriatric populations.

Knowledge of dysphagia management, cognitive rehabilitation, and communication disorders.

Familiarity with EMR/EHR documentation systems.

Work Schedule

Part-time (20-25 hours per week).

Monday through Friday with flexible scheduling.

No routine on-call or weekend responsibilities.
