Schedule: Monday - Friday, 8:00 AM - 5:00 PM EST (No nights, weekends, or on-call)
Position OverviewWe are seeking an energetic, proactive RN Clinical Coordinator to join our team in Sarasota, FL. This is a 100% office-based role that sits at the intersection of clinical care management, insurance operations, and team leadership.
In this role, you will act as the key operational bridge between adult patients, field nurses, collaborating providers, and insurance payers-ensuring seamless care plans, swift prior authorizations, and total regulatory compliance.
Note on Client Preference: This position requires full operational focus; candidates who currently own or operate their own private practice will not be considered.
Key ResponsibilitiesCare Coordination & Oversight: Review shift notes, collect/track physician orders, and manage a daily caseload of 20-30 adult patients receiving long-term care (LTC) and home health services.
Insurance & Authorizations: Handle insurance eligibility verifications, submit and track prior authorizations, and communicate directly with payers to prevent coverage gaps or service delays.
Referral Management: Review and process electronic client referrals coming in from hospitals, rehabilitation centers, and nursing facilities.
Interdisciplinary Liaison: Serve as the primary point of contact for collaborating physicians, clinical field staff, clients, and family members to keep care plans running smoothly.
Compliance & Audit Readiness: Ensure all clinical documentation aligns with Florida healthcare regulations, state/federal compliance standards, and internal quality assurance checks.
Setting: Dedicated office setting supporting home health and long-term care operations.

Licensure: Active Registered Nurse (RN) or Advanced Practice Registered Nurse (APRN) license in Florida.
Experience: 2+ years of recent experience in Clinical Coordination, Case Management, Long-Term Care (LTC), or Home Health.
Insurance Expertise: Direct, hands-on experience with prior authorizations, insurance verifications, and payer communications is required.

Tech & Software Skills:
Hands-on experience using PDFfiller to complete and manage Plans of Care (POCs).
Proficiency in operating Apple products / Mac environment.
Proficiency with EMR systems, healthcare documentation platforms, and medical software.
