Nurse Practitioner

Our organization is on the lookout for a dedicated Nurse Practitioner who is passionate about delivering primary and preventive healthcare to seniors participating in our PACE program. This role is integral to our interdisciplinary team, where you will:

• Conduct comprehensive assessments of patients
• Diagnose and manage both acute and chronic health conditions
• Prescribe necessary medications
• Create tailored care plans for individual patients

In this position, you will work closely with a diverse team that includes physicians, nurses, social workers, therapists, and caregivers, all aimed at promoting safe and independent living for our seniors.
