Job Summary
We are seeking a Board-Certified Family Medicine Physician to join a collaborative outpatient primary care team in San Francisco, CA. This position offers the opportunity to provide comprehensive, patient-centered care across all ages while emphasizing preventive medicine, chronic disease management, and continuity of care. The physician will work closely with a multidisciplinary clinical team to deliver evidence-based care, coordinate services, and improve patient outcomes through compassionate, high-quality medical care.
Roles & Responsibilities
Provide comprehensive primary care services to patients of all ages.
Diagnose and treat acute and chronic medical conditions.
Perform annual wellness visits and preventive health screenings.
Promote health maintenance, disease prevention, and patient education.
Manage chronic conditions through evidence-based treatment plans.
Coordinate patient care with specialists and interdisciplinary healthcare teams.
Utilize Electronic Health Records (EHR) to maintain accurate clinical documentation.
Order and interpret diagnostic tests and prescribe appropriate treatments.
Advocate for patients and coordinate cost-effective healthcare services.
Ensure compliance with clinical guidelines, quality standards, and regulatory requirements.
Qualifications
Board Certified in Family Medicine or Internal Medicine.
Active and unrestricted California Medical License.
Active DEA registration and NPI number.
Successfully completed an accredited residency program.
Minimum of 1-2 years of clinical experience preferred.
Strong communication, clinical decision-making, and patient-centered care skills.
