Exciting Opportunity: $20K Sign-On Bonus Available!Position: Home Health Physical Therapist

In this role, you will:

• Design and implement prescribed physical therapy treatments for patients in their residences, aiming for optimal physical function.
• Evaluate and document patients' physical capabilities to assist in physician assessments and inform treatment objectives.
• Create and execute a rehabilitation plan that aligns with the physician's treatment strategy and the goals set by the patient care team.
• Provide clear written care instructions to Physical Therapy Assistants (PTAs) and Home Health Aides, while supervising their performance and documentation.
• Accurately and promptly document patient observations, interventions, and evaluations.
• Communicate patient progress to the physician, Clinical Manager, care team, patient, and family, and ensure all necessary evaluations, treatment plans, progress notes, and discharge summaries are submitted.

Utilize Your Skills to Make a DifferenceRequired Qualifications:

• Degree from an accredited Physical Therapy Program recognized by the APTA.
• Current and unrestricted Physical Therapy license.
• Valid CPR certification.
• Experience working collaboratively with a healthcare team.

Preferred Qualifications:

• At least one year of experience as a Physical Therapist.

Compensation Details

Pay Range:

• $59.00 - $83.00 per visit/unit.
• Annual base salary between $93,000 - $128,000.

Additional Information

Tuberculosis Screening: This position involves direct patient interaction and is part of our TB screening program. If selected, you will need to undergo TB screening.

Driving Requirements: This role is included in our driver safety program, necessitating a valid state driver’s license and personal vehicle liability insurance. You must maintain vehicle insurance that meets or exceeds your state's minimum requirements, or at least $25,000 for bodily injury per person, $25,000 for bodily injury per event, and $10,000 for property damage, whichever is higher.
