Job Opportunity: Physical Therapist

Our organization is looking for a dedicated and empathetic Physical Therapist to become a part of our Healthcare & Medical Services team. This position is based on-site and focuses on delivering evidence-based rehabilitation services to our senior residents across various living arrangements, including independent living, assisted living, and memory care.

Key Responsibilities

• Conduct thorough assessments of residents' physical conditions.
• Design and implement personalized treatment plans tailored to individual needs.
• Develop and oversee fall-prevention and mobility enhancement programs.
• Collaborate closely with nursing and care staff to ensure comprehensive care.
• Provide education to residents and their families regarding treatment and wellness.
• Maintain accurate documentation of care within electronic health record (EHR) systems.
• Foster an environment that promotes safety, independence, and an enhanced quality of life.

Qualifications and Requirements

• Valid Physical Therapy license.
• Experience in geriatric care is preferred.
• Strong communication and interpersonal skills.
• Ability to work collaboratively within a multidisciplinary team.
