Position Overview

The Care Manager Assistant - LPN plays a vital role in facilitating the coordination of patient care and services. This position emphasizes high-quality care through a collaborative approach that involves coordinating, monitoring, and assessing services based on the needs and interactions with patients, all while adhering to relevant laws, regulations, and organizational standards.

Education, Experience, and Licensing Criteria
  • Completion of a nursing diploma (either an associate's or bachelor's degree in nursing).
  • Must hold a current and valid nursing license (LPN) in the relevant state, maintaining good standing throughout employment.
  • Preferred: At least one (1) year of clinical experience in a post-acute care environment.
  • Preferred: Previous experience in case management, utilization review, and discharge planning.
Job Responsibilities
  • Produce written communications that are clear, concise, and well-structured.
  • Exhibit strong organizational abilities, effectively prioritizing tasks and managing time.
  • Support the organization's goals and objectives while aligning with strategic plans.
  • Possess expert knowledge in the relevant field of practice.
  • Make independent decisions when necessary.
  • Understand the practices, procedures, laws, regulations, and guidelines applicable to post-acute care facilities.
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures.
  • Demonstrate proficiency in all competency areas, including general management duties, patient rights, and safety protocols.
  • Foster professional relationships with colleagues, vendors, and other stakeholders.
  • Ensure confidentiality of all proprietary and sensitive information.
  • Adhere to organizational policies, including those related to harassment and compliance.
  • Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training.
Key Functions
  • Utilize relevant computer systems efficiently, including data entry and retrieval.
  • Act as a liaison between external case managers, families, physicians, and community resources.
  • Provide training and education to patients, families, colleagues, and other care providers.
  • Implement care management standards, ethical practices, and coordination functions.
  • Deliver excellent customer service and maintain a positive demeanor towards patients.
  • Assist in the safe evacuation of patients when necessary.
  • Demonstrate reliable and consistent attendance.
  • Employ reasoning skills and sound judgment in various situations.
  • Collaborate effectively within an interdisciplinary team.
  • Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours.
  • Read, write, speak, and comprehend the English language proficiently.
