We're looking for a compassionate, resourceful Case Manager to support a 50-unit senior housing community in Buellton, CA.
This is a hands-on, on-site role where your work directly impacts people's ability to remain safely and successfully housed.
What You'll Be Doing

Pay Details: $62,400.00 to $72,800.00 per year

What You'll Be Doing
- Provide individualized case management based on each resident's needs
- Coordinate community resources (mental health, financial assistance, senior services, veteran resources, etc.)
- Collaborate with outside caseworkers, agencies, and community partners
- Support residents transitioning into housing after homelessness
- Help residents navigate challenges such as health needs, housing stability, and organization/hoarding concerns
- Support and facilitate community activities like meetings, groups, or on-site programs (gardens, classes, wellness events)
- Bachelor's degree (required) or Master's degree (MSW, MFT, or related, preferred)
- A minimum of 3 years of case management or related social services experience (depending on education level)
- Experience working with seniors, formerly unhoused individuals, or high-acuity populations is strongly preferred
- Comfortable working independently while collaborating with external partners
- Strong communication, organization, and follow?through skills
- Spanish speaking is a plus, but not required
