Under the guidance of the medical director, the Primary Care Physician (PCP) delivers essential medical services to participants of the PACE Organization (PO). This role encompasses evaluating patient needs, collaborating with the interdisciplinary team to formulate care plans, providing direct medical attention, and assessing the effectiveness of these care strategies. The PCP also engages collegially with other healthcare professionals, making autonomous decisions regarding medical requirements and health protocols, and performing necessary medical procedures.
Key Responsibilities:- Actively participate as a member of the interdisciplinary team (IDT) during initial, semi-annual, unscheduled, and annual evaluations; attend daily morning briefings and communicate any changes in participants' baseline conditions to relevant staff.
- Conduct initial and ongoing assessments of participants, develop care plans, and implement these plans. Ensure that the primary care treatment strategy aligns with the comprehensive care plan established by the interdisciplinary team.
- Refer participants to medical specialists as necessary. Oversee the care of participants in nursing facilities, conducting regular visits in collaboration with the IDT, and maintain communication with nursing home staff for ongoing monitoring of PACE participants on an individual basis; facilitate hospital admissions for nursing home participants when required.
- Manage and supervise participants' interactions with medical specialists and inpatient care, ensuring effective communication and follow-up regarding the outcomes of specialist visits.
- Engage as a member of the interdisciplinary team by attending meetings regularly, sharing updates on participant conditions, and collaborating on care planning to ensure 24-hour care delivery.
- At the request of the Medical Director, analyze medical outcomes, statistical data, and utilization metrics, including the necessity of hospitalizations and specialist referrals. Collaborate with the Medical Director to identify issues and implement best practices, working with clinical staff and the interdisciplinary team to enhance participant health and minimize unnecessary hospitalizations.
- Ensure accurate documentation and utilize appropriate diagnostic coding.
- Provide insights to the Medical Director on improving accountability of PO services to participants and referral sources, keeping the Medical Director informed about necessary resources as the program grows.
- Assist in developing policies, procedures, and standards of care; conduct ongoing evaluations of patient care practices and service delivery; offer guidance and training to staff on medical and quality assurance matters.
- Doctorate Degree (MD or DO) with a current and active physician license to practice in the state of PO. CDS Certification and DEA Registration are mandatory. Current BLS certification is required.
- Board Certification in Internal Medicine or Family Practice is essential; Board Certification in Geriatrics is preferred.
- A minimum of one (1) year of experience working with elderly or frail populations is required.
- A valid driver's license, proof of insurance, and reliable transportation are necessary.
- Must be proactive, possess business acumen, manage conflicts effectively, focus on customer service, demonstrate high-quality decision-making, and adapt to ongoing changes with organizational agility. Ability to work independently and exercise sound judgment is essential.
- Strong interpersonal skills to foster collaboration, commitment, and productivity when working with diverse teams, customers, and end-users. Comfort in a virtual, collaborative leadership environment is required.
- Exceptional written and verbal communication skills, tailored for audience comprehension. Ability to engage effectively with individuals and groups from varied backgrounds.
- Proficient in the Microsoft Office Suite (Word, Excel, PowerPoint, etc.) and capable of utilizing other software necessary for job functions.
- Excellent organizational abilities, capable of managing multiple tasks related to daily operations while maintaining high customer service standards. Ability to handle frequent interruptions and adapt to changing work priorities. Strong problem-solving skills are essential.
- Critical thinking skills and the ability to prioritize tasks effectively are required.
- This position may require occasional travel within the geographic region of the Centers.
