Our organization is looking to hire a full-time Medical Director/Physician for our Nursing Home facilities, dedicated to providing care in our nonprofit Skilled Nursing Homes located in the Capital Region of Upstate New York.

Situated in the Capital Region of Upstate New York, we offer a fulfilling and distinctive opportunity for a full-time Physician to become part of our nursing home medical team. Our facilities have established a strong reputation as a pioneering nonprofit provider, offering a wide array of services including nursing homes, subacute rehabilitation, independent retirement communities, assisted living, memory care centers, PACE, home health, hospice, and more. We are integrated within a comprehensive healthcare delivery system.
Position Overview:
This unique role involves collaborating with our nursing home medical team to care for our long-term care residents:
- Oversee 7 skilled nursing homes that provide a mix of long-term care, subacute rehabilitation, and memory care, with two locations designed as Green House models.
- Facilities are conveniently located within the Capital Region, with some just a few miles apart and others within a 20-minute drive.
- Join a highly cooperative medical team consisting of Physicians and Nurse Practitioners, along with on-site specialists in Podiatry, Psychiatry, Psychology, and Optometry.
- Benefit from a strong partnership with the Albany College of Pharmacy, recently launching a Geriatric Pharmacotherapy Service.
- Receive substantial leadership support from the Chief Medical Officer, Chief Nursing Officer, Director of Quality & Safety, and Vice President for Residential Services, all of whom bring extensive experience in long-term care and a commitment to our mission of caring for residents and colleagues alike.
- Engage in teaching opportunities with Albany Medical College’s Geriatric Fellowship Program, fostering the development of geriatric-trained Physicians and Nurse Practitioners.
Qualifications:
- Board Certification in Internal Medicine or Family Medicine is required.
- Certification in Geriatric Medicine or Hospice & Palliative Care is preferred but not mandatory.
- Certification as a Medical Director (CMD) is preferred but not essential.

COMPENSATION PACKAGE
We offer a comprehensive salary and benefits package that includes:
- A starting compensation of $278,000.
- The stated salary represents the initial range of total compensation and does not account for productivity bonuses, quality and performance incentives, extra shift bonuses, and other applicable cash compensation. Additionally, providers typically receive a CME allowance and other benefits from their medical group employer. Please note that this salary range is provided in accordance with state law and may vary based on the factors mentioned.
- A starting bonus of $30,000.
