Job Description

The Nurse Coordinator is a key role within our live-in nursing agency, responsible for managing all aspects of client placements, nurse coordination, and clinical quality assurance. Working closely with your colleagues the Registered Manager and Care Advisory Team, you will oversee the end-to-end delivery of personalised nursing care in clients' homes, ensuring safety, continuity, and a consistently high standard of service. The core focus of our nursing team is to provide the highest quality nursing care to those who wish to receive healthcare in the comfort of their own home instead of receiving this care in a hospital or community setting. Many of our clients receiving nursing care at home are nearing the end-of-life so we pride ourselves on providing a sensitive, compassionate and dignified pathway, and offering much-needed emotional and practical support to all the family, ensuring our clients are matched with an appropriate nurse, and placements are well co-ordinated and managed to ensure the best possible nursing care is delivered that meets clinical and regulatory guidance and standards Key Responsibilities : Client Onboarding & Service CoordinationRespond to new client enquiries and promote the benefits of our live-in nursing servicesComplete in-depth Care Needs Assessments with new clients, ensuring accurate clinical documentationMatch clients with appropriately skilled nurses based on clinical needs, personality fit, and availabilityPrepare and hand over care assessments to nurses prior to placement, ensuring smooth transitionsMaintain regular communication with clients and families to monitor satisfaction and address concernsLiaise with external professionals including GPs, hospital discharge teams, hospice staff, and community nursesNurse Coordination & SupportMaintain up-to-date nurse availability records and manage bookings efficiently to ensure continuity of careMonitor all live-in placements, providing support and guidance to nurses as neededEnsure nurses are fully equipped with required documentation, PPE, and medical kits before starting placementsConduct regular check-ins and act as a point of contact for nurses in placementRecruitment & InductionHandle all new nurse enquiries and manage the recruitment process from application to interviewConduct screening, interviews, and manage all necessary pre-employment checks and referencesEnsure all new nurses have the relevant training, ensuring they are prepared for live-in placementsMonitor nurses' compliance with NMC standards and provide support with revalidation and annual reviewsCompliance & Quality AssuranceAudit all clinical documentation, including care plans, risk assessments, and nurse weekly summariesEnsure all placements meet regulatory requirements, internal standards, and care quality expectationsMaintain accurate records relating to nurse training, availability, and placement historySupport ongoing training and development of nurses by sharing clinical resources and updatesOn Call and Out of Hours SupportParticipate in the on-call rota to support nurses and clients outside of office hoursProvide timely clinical advice and problem resolution during out-of-hours periodsEscalate urgent issues as necessary to ensure client safety and nurse wellbeingAdministrative & Operational DutiesManage all relevant documentation related to client care and nurse bookingsAssist with updating nurse profiles and creating client-facing nurse introductionsContribute to marketing and promotional efforts, including reviewing website content or attending eventsCollaborate with internal teams (IT, HR, Compliance) to support continuous service improvementQualifications: Registered Nurse with current NMC registrationStrong communication skills for effective client interactionExperience in developing and implementing care plansProficient in IT systems relevant to nursing and patient careExcellent organizational skills to manage multiple tasks and prioritiesAbility to work collaboratively within a healthcare teamConsidering both full and part-time candidates for this role.
