Position Summary\n
\n The School Physical Therapist will be assessing students' physical development level; providing appropriate treatment to meet individualized student objectives; providing recommendations for program development and student placement; and providing direction to Physical Therapy Assistants. We're searching for a PT who is eager to collaborate with a multidisciplinary team including OTs, SLPs, and other ancillary staff. Responsibilities\n

• Hours per week: 37.5 hours per week
\n
• Contract length: School Year
\n

Key Duties\n

\n
• Interpret physician orders related to professional area of responsibility
\n
• Assess gross motor skills and therapeutic equipment needs of students
\n
• Develop, implement, monitor and modify treatment plans based on physician orders and assessment of student as appropriate
\n
• Collaborates with the school team to develop Individualized Education Programs and Section 504 Plans to address the students' unique needs
\n
• Consults with teachers, parents, other personnel, and/or outside professionals for the purpose of providing requested information, developing plans for services, and/or making recommendations
\n
• Coordinates with community agencies including medical, judicial, social service, and mental health services for the purpose of determining appropriate treatments, etc
\n
• to meet the needs of specific students
\n
• Participates in meetings, workshops, and seminars as assigned for the purpose of conveying and/or gathering information required to perform functions
\n
• Prepares a wide variety of written materials (e.g. evaluations, correspondence, memos, treatment plans, reports, etc.) for the purpose of documenting activities, providing written reference, and/or conveying information
\n
• Complete IEP, MET, Medicaid, and treatment forms/reports
\n
• Maintain treatment/progress notes and Medicaid documentation per professional standards
\n
• Train Paraprofessionals/PTAs on various student plans
\n
• Regular and predictable attendance
\n

Requirements \n
Licenses & Certifications\n
• Licensed Physical Therapist – State of Michigan\n
• Michigan Driver’s License\n
• Pediatric experience preferred\n
