Key Responsibilities

• Design and implement prescribed physical therapy treatments for patients in their homes, aiming for optimal physical function.
• Evaluate and document patients' physical capabilities to assist in physician assessments and inform treatment objectives.
• Create and execute rehabilitation programs aligned with the physician's treatment plan and the goals of the patient care team.
• Provide clear written instructions to Physical Therapy Assistants (PTAs) and Home Health Aides, supervising their performance and documentation.
• Record patient observations, interventions, and evaluations promptly and comprehensively.
• Communicate patient progress to the physician, Clinical Manager, care team, patients, and their families, while submitting all necessary evaluations, treatment plans, progress notes, and discharge summaries.

Essential Qualifications

• Degree from an accredited Physical Therapy Program recognized by the APTA.
• Current and valid Physical Therapy license.
• Current CPR certification.
• Experience working collaboratively with a healthcare team.

Preferred Qualifications

• At least one year of experience as a Physical Therapist.

Compensation

The pay structure is as follows:

• $59.00 - $83.00 per visit/unit
• Annual base salary ranging from $93,000 to $128,000

Additional Information

• TB Screening: This position involves patient interaction and is part of our Tuberculosis (TB) screening program. Candidates selected for this role will need to undergo TB screening.
• Driving Requirements: This role is included in our driver safety program, necessitating a valid state driver’s license and personal vehicle liability insurance. Candidates must maintain insurance that meets or exceeds state minimum requirements, or at least $25,000 for bodily injury per person, $25,000 for bodily injury per event, and $10,000 for property damage.
