As a therapist within our organization, you will report directly to the Clinical Manager and play a vital role in assisting patients to regain their strength, mobility, and independence from the comfort of their own homes. By providing tailored care that emphasizes functional improvement, you will empower individuals to overcome challenges and enhance their quality of life.

Key Responsibilities- Design and implement prescribed physical therapy treatments for patients in their homes, aiming for optimal physical function.
- Evaluate and document patients' physical capabilities to assist in physician assessments and inform treatment objectives.
- Create and execute rehabilitation programs aligned with the physician's treatment plan and the goals of the patient care team.
- Provide clear written instructions to Physical Therapy Assistants (PTAs) and Home Health Aides, supervising their performance and documentation.
- Record patient observations, interventions, and evaluations promptly and comprehensively.
- Communicate patient progress to the physician, Clinical Manager, care team, patients, and their families, while submitting all necessary evaluations, treatment plans, progress notes, and discharge summaries.
- Degree from an accredited Physical Therapy Program recognized by the APTA.
- Current and valid Physical Therapy license.
- Current CPR certification.
- Experience working collaboratively with a healthcare team.
- At least one year of experience as a Physical Therapist.
The pay structure is as follows:
- $59.00 - $83.00 per visit/unit
- Annual base salary ranging from $93,000 to $128,000
- TB Screening: This position involves patient interaction and is part of our Tuberculosis (TB) screening program. Candidates selected for this role will need to undergo TB screening.
- Driving Requirements: This role is included in our driver safety program, necessitating a valid state driver’s license and personal vehicle liability insurance. Candidates must maintain insurance that meets or exceeds state minimum requirements, or at least $25,000 for bodily injury per person, $25,000 for bodily injury per event, and $10,000 for property damage.
