Nurse Home Medical Director - Physician - Cohoes, NY

Our organization is in search of a dedicated and forward-thinking Physician Medical Director to oversee medical services at Eddy Village Green, located in Cohoes, NY. This facility is a nationally acclaimed nursing home that has been a pioneer of the Green House model in New York State.

This role offers flexibility, welcoming both full-time and part-time physicians who are committed to person-centered care and innovative approaches to elder care.

• 16 ranch-style homes, each accommodating 12 residents in a nurturing, family-oriented atmosphere.
• A non-institutional philosophy that prioritizes autonomy, dignity, and meaningful connections.
• A collaborative care team comprising physicians and nurse practitioners, along with on-site specialists in Podiatry, Psychiatry, Psychology, and Optometry.

In the role of Medical Director, your responsibilities will include:

• Managing and coordinating medical care throughout the campus.
• Developing and assessing resident care policies in accordance with current best practices.
• Working alongside nursing, pharmacy, therapy, and administrative teams to ensure integrated, high-quality care.
• Engaging in Quality Assurance and Performance Improvement initiatives.
• Acting as a clinical leader and mentor, fostering excellence in geriatric care.

• Flexible working hours.
• Open to both full-time and part-time applicants.
• Average of 12-14 resident visits per shift, including routine and sick visits.
• On-call responsibilities every 6-8 weeks (phone calls only for a week).
• Extensive support services available within the facility, including specialists in Pulmonary, Physiatry, Psychiatry, WOCN, Podiatry, and more.

• Board Certification in Internal Medicine or Family Medicine.
• Preferred but not mandatory: Certification in Geriatric Medicine or Hospice & Palliative Care.
• Preferred but not mandatory: Certified Medical Director (CMD) credential.
• A strong passion for providing care to older adults in a collaborative, mission-driven setting.
