You will have the time to build trusted relationships, understand each person's individual needs and support them to live safely and independently at home.The roleDepending on the individual care package, your responsibilities may include:• Delivering safe and person-centred renal care within the client's home• Monitoring the client's condition and responding appropriately to any changes• Supporting dialysis-related care in line with the agreed clinical care plan• Administering medication and maintaining accurate clinical records• Working closely with families, healthcare professionals and the wider NursePlus clinical team• Providing reassurance, independence and continuity for the person receiving careAbout youYou will need to be:• A Registered Nurse with a valid NMC registration• Confident working independently in a community setting• Calm, dependable and clinically assured• Committed to delivering safe, respectful and person-centred care• Able to travel within the Cornwall areaPrevious renal or dialysis experience would be highly valuable.
