We invite you to join our team in delivering primary medical care in partnership with our respected PACE Organization (PO) Physicians. As a full-time Nurse Practitioner, you will be responsible for conducting assessments, creating care plans, and ensuring that participants receive thorough care in a nurturing and supportive atmosphere.

Your Responsibilities Include:

  • Working alongside PO physicians to perform assessments and execute care plans.
  • Engaging in discharge planning and providing treatment for acute illnesses.
  • Conducting home visits as needed.
  • Assisting in the development of clinical policies and procedures.
  • Participating in the provider on-call rotation.

Essential Qualifications:

  • Master's degree in Nursing.
  • 5-7 years of relevant experience, ideally in long-term care settings.
  • Current license to practice in North Carolina.
  • Certification in CDS, DEA Registration, and Board Certification are mandatory.
  • Current BLS certification.
  • Willingness to travel within the geographic area of the Center.
