Our organization operates Skilled Nursing Facilities (SNFs) that offer:

• Long-term care for residents needing ongoing skilled support and compassionate daily assistance
• Short-term, sub-acute rehabilitation for patients recovering from surgery, illness, or injury

Position Summary

The Speech Language Pathologist (SLP) delivers direct rehabilitation services in speech therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.

Qualifications and Experience

• Graduation from an accredited speech therapy program (minimum MS or MA in SLP)
• ASHA certification (CCCs) or eligibility for a 9-month Clinical Fellowship Year (CFY) to obtain ASHA certification
• Active registration/licensure in the applicable state, with maintenance of good standing throughout employment
• Preferred: One year of experience in post-acute care or a related field
• CPR certification required upon hire or to be obtained during orientation, with ongoing maintenance throughout employment

Specific Job Responsibilities

• Engage in community health initiatives as appropriate
• Proficient in Microsoft Word, Excel, and email communication
• Coordinate with patients, families, and support departments to effectively address patient needs
• Exhibit proper body mechanics at all times
• Demonstrate the ability to make independent decisions when necessary
• Knowledgeable about practices, procedures, and regulations governing post-acute care facilities
• Implement and interpret departmental programs, goals, objectives, policies, and procedures
• Maintain proficiency in all competency areas, including patient rights, safety, and sanitation
• Foster professional relationships with colleagues and vendors
• Ensure confidentiality of all proprietary and sensitive information
• Adhere to company policies, including those related to harassment and compliance
• Exhibit integrity and professionalism by following the organization’s Code of Conduct and completing required compliance training

Essential Functions

• Establish, assess, and adjust realistic, measurable, and functional goals (SLP CCC only)
• Supervise and evaluate care provided by SLP CFYs (SLP CFY only)
• Follow speech therapy treatment plans under the guidance of the supervising SLP CCC, in compliance with Federal and State regulations
• Document patient progress accurately and in a timely manner
• Utilize therapy software effectively
• Provide excellent customer service and maintain a positive attitude towards patients
• Assist in patient evacuation when necessary
• Demonstrate reliable attendance
• Employ reasoning skills and sound judgment in decision-making
• Collaborate effectively within an interdisciplinary team
• Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, walking, and reaching throughout working hours
• Read, write, speak, and comprehend the English language
