- We are looking for an experienced Senior Social Worker to join our Hospital Discharge and Home First Service, supporting adults to leave hospital safely, regain independence and remain connected to their communities. \n
- \n
- Working within University Hospital Southampton (UHS), one of the region's largest and busiest acute hospitals, you will play a key leadership role in supporting frontline practice, facilitating complex decision-making and driving excellent social work standards across the service \n
- \n
- As a Senior Social Worker, you will provide professional leadership and expert practice support to social workers and social care practitioners within a busy multidisciplinary hospital setting. \n
- \n
- Working closely with health colleagues, community teams, voluntary sector partners and families, you will support timely and effective hospital discharges while ensuring people's strengths, wishes and outcomes remain at the centre of decision-making. \n
- \n
- This role combines professional leadership, practice development, supervision and quality assurance, alongside maintaining oversight of complex cases where required. \n
- \n
- Provide professional leadership, coaching and guidance to Social Workers and Social Care Practitioners. \n
- \n
- Support staff to deliver strengths-based, person-centred assessments and outcome-focused support planning. \n
- \n
- Promote best practice in Care Act assessments, Mental Capacity Act decision-making, safeguarding and risk management. \n
- \n
- Support safe and timely hospital discharges in line with Home First principles. \n
- \n
- Provide reflective supervision and mentoring to practitioners, students and newly qualified social workers. \n
- \n
- Support practitioners with complex casework, safeguarding enquiries and legal literacy. \n
- \n
- Undertake audits, quality assurance activity and practice improvement work. \n
- \n
- Work collaboratively with health, housing, community and voluntary sector partners. \n
- \n
- Deputise for the Team Manager when required. \n
- \n
- Champion continuous improvement and contribute to service development. \n
- \n
